Platinum Sponsors:
A sponsorship fee of $10,000 or more. You may combine sponsorship opportunities to equal $10K or contribute to a specific function. We recommend that you contribute to the activities with the highest attendance such as receptions and luncheons. The platinum sponsorship will include a 10 x 10 booth free of charge (additional space is available for a fee). Your company's logo will appear in strategic locations throughout the foyer of the Albuquerque Convention Center and at the activity in which you have sponsored. In addition, your company will be recognized in the advance flyer announcements, during the sponsored event, in the final program, and on the conference Web site.

Gold Sponsors:
A sponsorship fee of $7,500 or more. You may combine sponsorship opportunities to equal $7.5K or contribute to a specific function. The Gold sponsorship will include a 10 x 10 booth at a reduced fee of $1000 (additional space is available for a fee). Your company's logo will appear in strategic locations throughout the foyer of the Albuquerque Convention Center and at the activity in which you have sponsored. In addition, your company will be recognized in the advance flyer announcements, during the sponsored event, in the final program, and on the conference Web site

Silver Sponsors:
A sponsorship fee of $5,000 or more. You may combine sponsorship opportunities to equal $5K or contribute to a specific function. The Silver sponsorship will include posting of your company's logo will in strategic locations throughout the foyer of the Albuquerque Convention Center and at the activity in which you have sponsored. In addition, your company will be recognized in the advance flyer announcements, during the sponsored event, in the final program, and on the conference Web site. The Exhibitor Booth fee will not be waived.

Bronze Sponsors:
A sponsorship fee of $2,500 or more. You may combine sponsorship opportunities to equal $2.5K or contribute to a specific function The Bronze sponsorship will include posting of your company's logo in strategic locations throughout the foyer of the Albuquerque Convention Center and at the activity in which you have sponsored. In addition, your company will be recognized in the advance flyer announcements, during the sponsored event, in the final program, and on the conference Web site. The Exhibitor Booth fee will not be waived. .

Sponsorship Registration Form

We have a number of opportunities for Congress sponsorship, which are outlined below.
We ask that you prioritize you preferences for sponsorship.

1. Vendor Booth Exhibit:
This will include a 10x10 booth space at the Convention Center Exhibitors Area and Cyber Cafˇ. Curtains and skirted table with chairs are included. Additional costs for electricity phone service and Internet services will apply.

2. Additional Booth Space
This will include an additional 10x10 booth space at the Convention Center Exhibitors Area and Cyber Cafˇ. Curtains and an additional skirted table with chairs are included. Additional costs for electricity phone service and Internet services will apply.

3. Attendee Portfolio Bag Stuffing
USNCCM7 is offering your company the opportunity to have a flyer or small brochure inserted in each attendee portfolio bag. Your flyer or small brochure will be distributed to all USNCCM Conference attendees. Your sponsorship also includes recognition in the final conference program, advance program and on the conference Web site. Note: Flyers must be received at USNCCM7 headquarters no later than 1 July 2003.

4. Luncheon Sponsorship
This sponsorship opportunity puts your company in the spotlight during the lunch for one day of the Congress. Between 800-1000 attendees, including the USNCCM7 Scientific Committee, volunteer leaders, and deans from across the country, annually attend this luncheon. Your company's logo will appear in strategic locations throughout the foyer of the Albuquerque Convention Center. In addition, your company will be recognized in the final program, advance program and on the conference Web site. Requests will be handled on a first-come, first-served basis.

5. Advance Program Advertising
Your company's full-color advertisement will appear on the back cover of the 2003 USNCCM7 Conference & Exposition's Advance Program. This program will be distributed to all USNCCM7 attendees.

6. Reception Sponsorship
The Reception is held on Sunday evening from 6:00 to 9:00 in the Ballroom B, next to the Exhibitors Area, and is one of the most popular events held during the USNCCM7 Congress. Your company will be an instant hit with both attendees and exhibitors by sponsoring this event. No sessions are held during this time, to allow both attendees and exhibitors ample time to relax and interact with exhibitors, sponsors and colleagues. In addition, your company's sponsorship will be recognized in the final conference program, the advance program and on the Web site. Requests will be handled on a first-come, first-served basis.

7. Theme Refreshment Break
Sponsor one of the events that attendees look forward to each day. After listening to educational sessions all morning, attendees relish the opportunity to relax and mingle with their colleagues. The refreshment breaks take place in the Exhibitors Area of Albuquerque Convention Center and will be open to all attendees and exhibitors. In addition, your company will be recognized with signs throughout the convention center and exposition hall, as well as in the final program, advance program and on the conference Web site. Requests will be handled on a first-come, first-served basis.

8. Annual Awards Banquet
The USNCCM7 Awards Banquet is the final official event of the 2003 Congress and is your chance to leave a lasting impression attendees will take with them until the 2005 Congress. This year marks the 7th Annual Awards Banquet. During the Banquet the USACM Awards will be presented. Your company's name and logo will appear on signage throughout the reception and at the banquet. Your sponsorship will include entertainment during the banquet. In addition your company will be recognized in the final conference program, advance program, awards banquet program and menu and on the conference Web site. Requests will be handled on a first-come, first-served basis.

9. Annual Awards Banquet Reception
This unique sponsorship opportunity offers your company the chance to stand apart from the competition by sponsoring the reception given to honor the 2003 USNCCM7 Congress award winners. Your company's name and logo will appear on signage throughout the reception. In addition your company will be recognized in the final conference program, advance program, awards banquet program and menu and on the conference Web site. Requests will be handled on a first-come, first-served basis.

10. Conference Proceedings
The official 2003 USNCCM7 Annual Conference Proceedings are the collection of all the abstracts of papers presented by conference speakers. Any USACM member who does not attend may request a free copy for a limited time after the conference. The conference proceedings are a valuable reference tool that attendees, members and nonmembers use over and over again. This sponsorship places your company's logo on the proceedings itself. In addition, your company's sponsorship will also be recognized in the final conference program, advance program and on the conference Web site. Requests will be handled on a first-come, first-served basis.

11. Conference Proceedings Workstations
Your company provides computers for the very popular Cyber Cafe. These computers are very popular among attendees trying to plan how they will spend their valuable time. These computers will be strategically located near the Exhibitors area of the Albuquerque Convention Center at the Cyber Cafe. In addition, your company's sponsorship will be recognized in the final conference program, advance program and on the conference Web site. Requests will be handled on a first-come, first-served basis.

Note: This sponsorship opportunity can be combined with the Internet Access Screensaver sponsorship opportunity.

12. Internet Access and Screensaver
Your company's logo will appear as a screensaver on computers dedicated to giving all attendees Internet access during the conference. The Internet computers will be located in the exhibit hall at the Cyber Cafe. Conference attendees will thank your company for providing them with the means to keep in touch with the office and family while at the USNCCM7 Congress. In addition, your company's sponsorship will be recognized in the final conference program, advance program and on the conference Web site. Requests will be handled on a first-come, first-served basis.

Note: This sponsorship opportunity can be combined with the Conference Proceedings Workstations available in the lobby of Albuquerque Convention Center.
